New Patient Registration

To register at the practice you need to live within the practice catchment area as outlined in the practice boundary map below, you then have 2 options.

We only ask you to provide your ID and proof of address to avoid any identity fraud.

If you recently moved to our catchment area but don’t have the documents, we will still process the registration.

How To Register?

Our patient registration policy does not discriminate on the grounds of race, gender, social class, age, religion, sexual orientation or appearance, disability or medical condition.

To register as a new patient at the practice. Please follow the registration process below.

Please allow 12 working days for your Registration to be processed. If you need to be seen urgently you can be seen as an (Immediate & Necessary Patient)

You will need to be able to provide proof of ID that is valid within the last 3 months if you are moving GP to GP.

Examples of documents we accept are as follows:

  • Photo ID (Passport or Driving Licence)

Proof of Address in the form of:          

  • Utility bill (Gas/ Electric/Telephone)
  • Bank/credit card statement
  • Tenancy Agreement
  • Benefit/Family credit letters
  • Council Tax Bill (Valid within the year)

Check You Live Within The Boundary Area

Simply enter your postcode to check to see if you are living within our catchment area.

If you are already a patient and are about to move to a new address please let the patient service team know by completing our change of details form. They can decide whether or not you are still within our catchment area. If you are not, and you would like to stay with the practice, please let us know. All cases will be dealt with on an individual basis, by the practice manager and your named doctor.

Please Note: We cannot register anyone under the age of 16 without an Adult being registered. Please see option 2 for more information.

Other Ways to Register

If you live within the boundary area, and the forms shown above are not applicable to you please use the following forms.

You can ask reception for a paper Registration and Practice Health Questionnaire if you struggle to use our online facility. 

For identification purposes, you will need to upload/show proof of ID and proof of address at the end of the form. Proof of ID can be a passport or driving licence. Proof of address can be a utility bill (gas, electricity or water) or bank statement – dated within the last 3 months.

All registration applications must be submitted with your NHS number, applications submitted without NHS number may be rejected. If you are unsure whether you have this, please refer to this guidance.

Book a New Patient Health Check

Following your registration, adults will receive a letter from the surgery inviting you to book a New Patient Health Check with our Healthcare Assistant or one of our Practice Nurses.

Please Note: This does not apply to Under 16’s.

NHS England requires that all registered patients have an allocated Doctor within the practice. This Doctor is the named person responsible for your care however you can continue to see whichever clinician you choose.

Temporary Services

If you are ill while away from home or if you are not registered with a doctor but need to see one you can receive emergency treatment from the local GP practice for 14 days. After 14 days you will need to register as a temporary or permanent patient.

You can be registered as a temporary patient for up to three months. This will allow you to be on the local practice list and still remain a patient of your permanent GP. After three months you will have to re-register as a temporary patient or permanently register with that practice.

To register as a temporary patient simply contact the local practice you wish to use. Practices do not have to accept you as a temporary patient although they do have an obligation to offer emergency treatment. You cannot register as a temporary patient at a practice in the town or area where you are already registered.

Your Named GP

Patients are registered with a named doctor at the practice, but you are free to see whichever doctor or nurse you wish. We will always try to allocate to your chosen clinician, but you may have to see another doctor or nurse when no appointments are available.
