Out of Area Registration – Important Information

Forest Edge Practice is pleased to announce the expansion of its patient catchment area, enabling more local residents to access quality NHS healthcare closer to home.

The change, effective immediately, allows individuals and families living in newly included areas to register with the practice and benefit from its comprehensive primary care services.

You can check if your address falls within our updated catchment area by entering your postcode on our website or please visit this link and we will endeavour to get back to you.

However, we understand that some patients still reside outside of our updated catchment area and wish to register or remain registered as ‘Out of Area’ patients. If this applies to you, please read the following information carefully before making your decision.

As you reside outside of our practice boundary, it is important that you understand the Out of Area policy:

  • As an Out of Area patient, we are not obligated to provide home visits, and there may be limitations regarding your access to local community services, such as adult social care and community nursing.
  • In the event of an urgent medical condition or injury that prevents you from attending our practice in person, you will be advised to contact NHS 111. This service will help you access appropriate local care, which may include a face-to-face appointment or, if required, a home visit.
  • If it is determined that you are experiencing complex or ongoing health issues that require support from local community services, we may recommend that you register with a GP practice closer to your home.
  • Please note: If at any point we feel that your clinical needs would be better supported by a practice nearer to where you live, we may review your Out of Area registration and advise you accordingly. Our priority is to ensure that all patients receive safe, accessible, and appropriate care.

Disclaimer form is available on our website under the Forms section.
